Applications may be picked up at the B.B. King Museum and Delta Interpretive Center / Lucille’s Gift Shop Monday through Saturday between the hours of 10:00 a.m. – 5:00 p.m.
Applications may also be found and printed below. You MUST fill out all three form.
Please also bring a copy of the participant’s birth certificate when dropping off the application.
